Junior Golf Camp Participation Waiver and Release of Liability

Assumption of Risk and Waiver of Liability

I, the undersigned parent or legal guardian of the camper listed above, hereby give permission for my child to participate in the Junior Golf Camp held at Village Green Golf Club. I understand that participation in sports and outdoor activities, including but not limited to golf, involves certain inherent risks of injury.

I acknowledge and fully understand that my child will be engaging in physical activities that may involve risk of injury which may result from, but is not limited to, slips, falls, contact with equipment, other participants, or weather-related conditions. I assume all risks and hazards incidental to my child’s participation in the golf camp.

Medical Authorization

In the event of an emergency, I authorize Village Green Golf Club staff to administer first aid and/or secure emergency medical care for my child as deemed necessary. I understand that all reasonable efforts will be made to contact me before such action is taken.

Release of Liability

I hereby release, waive, and discharge Village Green Golf Club, its owners, staff, instructors, volunteers, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by my child while participating in camp activities or while on the premises.
